I thought it odd coming from you @Randy, but saw it as a challenge that adds to the intent of the thread whatever the case was .

I figured, this subject is a sore spot with me, Sidney and a ton of others because we see these works of fiction all too frequently and the damage they can cause.

but a consultant has stated that their should be an authorization signature

When confronted by consultants or auditors who impose signatures on work instructions or documented procedures, ask whether s/he has an official copy of the standard with signature/s on it.
