SAME QUESTION AS ABOVE JUST ADDED A LITTLE AT THE END

I am working with a customer who is running QS9000. For him to put my product in the line he needs me to be at leastPPAP level 2. Is this a form I can answer questions to or is there more to it? We are not ISO- QS or anything, but customer does not sound like I have to be, just that I have to supply the info on this oparticular item.

Thanks for any help or direction! I have found a quality manual on this site for QS but it seems like there is a question and answer form for the PPAP that is not included?

You need to supply the following:
Warrant- document that shows that the part complies, for a copy download Elsmar.com/pdf_files/Partcertall-6.06.xls
Samples and dimensional reports.
Material certificates.
You should have other supporting evidence at your plant such as a control plan, flow chart of the process.
