We state attachments as supplemental forms, data or material used for clarification or explanation. Regarding these attachments to a procedure or work instruction, can the attachment be of a different rev level and effectivity date? Say in the case of using a form? Thanks, Bryan IP: Logged |

If you are asking what I think you are - "Do forms or other attachments always need to carry MATCHING rev levels & effective dates - as their parent work instruction & procedure? IE: Do you need to advance rev/effective date of attachments/forms with each procedure/WI change? I would think - "not necessarily" - the WI or Procedure could carry an advanced rev or date identification when the form/attachment is not part of a change/update -- BUT if the method of identifying & controlling document changes in (4.5) is such that "all" form/attachment rev levels & dates will be advanced with any change to procedure & WI then you would need to update the rev levels on each form/attachment every time a change is made to the "parent document. What works for us is to simply state the "form or attachment" identifying number in the parent document - then identify rev levels & effective date changes to them in the master listings for "forms"

If you revise the procedure and the form is not affected there is no reason to 'rev' the form. You only 'rev' the form when you revise the form. IP: Logged |
