Need to understand "recommendations" and "recommended corrective action". If it is a recommendation then it is a suggestion and not a corrective action? Where and how in an 8D report should this be documented or should it even appear in the report?

Re: Recommendations

If you take the term "action" a recommendation isn't an action! 8D's are about actions. "We did this, we did that", not "we recommend this or that"...

Simples!

Thanks and agree. What is the purpose and value of sending an 8D report to the client with recommended actions or corrective actions that are outstanding/ not been completed yet? Surely a full report with all actions completed is required?

Are you asking your client to take corrective action?

If so their root cause removal should be requested not recommended.

But did your corrective action team include the client to make the request unnecessary?

Hi John. Our organisation needs to take corrective action and then need to submit a report to the client. The Project Manager proposed we give client recommendations AND the few corrective actions are still open.

Matrix,

Then your manager's proposal is wrong.

Report what your organization has done to correct the problem to the satisfaction of the client and report what you've done to the remove root causes to stop recurrence.

Report how you are verifying the effectiveness of your actions.

Both "proposal" and "recommendation" are tentative or weak and will undermine your client's confidence in your company.

Please remember that quality assurance is defined as providing confidence that your client's requirements will be fulfilled.

Best wishes,

John

Thank you John. The closed actions will then verify the effectiveness...

1) Although recommendations and proposals are weak, often in the aviation industry these are given in investigations e.g. safety aspects etc. When this is done then the root cause is not removed?
2) Often corrective actions can take a long time, can an 8D report then not be issued with open corrective actions?

I completely agree most of the time corrective actions take a long time than estimated due to several technical and non-technical reasons. But, a corrective action could be issued with open action items and there should be a planned verification of effectiveness in that case, where customer is aware that the team is working on the solution. It is totally debatable and depends on the problem definition.
