FMEA 4th Edition Blank P and D FMEA sheets in Excel .xls spreadsheets

hi, recently I show my FMEA's (D and P) and I got from the auditor the comment that I'm mixing in process with product stuff. Can some one provideme an example (template) of both FMEA's?

Generally the best source for automotive work is the AIAG FMEA manual. You might also find examples here by clicking on the "Post Attachment Files" tab at the top of the page and doing a search.
