Guidelines in writing an audit report and determing the best auditor

Hi,

I would like to ask your help. Is there any guidelines how to write an audit report? can you share it with me? like for instance the audit report should contain the ff or should answer the followng?

1. what records you saw?
2. what findings you have seen?
3. what is the refence number?


and I do have another question:

As lead auditor how do you know who is the best auditor? meaning do youhave any indicator for instance:

1. attending meeting (yes or no)
2. effective in writing audit report (yes or no)

The best advice I can give without writing a book here is for you to obtain a copy of ISO 19011:2002. Audit reports and auditor competence are specifically covered.

Have you attended a Lead Internal Auditor course, mjflkitty? The course should answer answer your question on the format or structure of the audit report and the purpose of the audit report (which should answer your question about what to include).

Let me ask you this, mjflkitty...if you read an audit report that just said that everything was fine and met the requirements but listed no evidence/proof that support such a claim, would you accept it? How could you believe that all was acceptable if there is no proof?

I'm not sure I understand how meeting attendence is an indicator of a good auditor, but I use the following criteria to assess my internal auditors:

  • Comprehension of our internal audit process
  • Comprehension of ISO 9001 standard (in other words, can rephrase questiosn and the requirements such that the employee understands what is being asked)
  • Takes detailed notes (e.g., document rev level, id, name, employees spoken with, etc.)
  • Adherence to schedule
  • Investigative
  • Adheres to safety rules
  • Listening skills
  • Attitude (i.e., non-confrontational, pleasant)

Just received word that I passed the RAB accreditied ISO9001:2000/TS 16949:2002Lead Auditor course and exam. :D

Agree with above, plus document clauses and SOP Numbers in any non-conformances and summary reports, along with titles of those contacted during audit. Also, if there is no non-conformance, don't keep trying to find one, move on. The audit is to determine compliance.

I haven't attended a Lead Auditor course but i have attended a n internal auditor course. the company said i will not be taking the lead auditor course since i'm only auditing inside the company but i'm thier lead auiditor. :eek:

I always tell my auditors to have a comprehensive audit report with an objective evidence which is my two indicator for good audit report but then i'm still want to know what other guidelines i can give to them?

What would be the basis for this one? do you think this would sound subjective?

Hi,
I was 'witness audited' for RVA accreditation of a certification body, and the + point which the 'witness auditor' mentioned in his concluding report about my auditing skills was the 'depth to an audit thread'.

He had also mentioned my skill and desire of ensuring assurance of customer requirements / focus, during my keeping of audit trails for a constructive conclusion into an objective evidence for a conforming or non conforming state .
i.e. comprehension of process(es) followed and the skill of framing questions.

But the question of evaluating from the assessment report is different from one where the lead and auditor are together watching each other ( say, witnessed)

Are the two courses ISO9001:2000/TS 16949:2002 held together? Or that was a consecutive 10 day programme for both?

Any way passing an exam is always reassuring and certifying for your quality inputs. congratulations.

Thanks for the congrats. This was a 5 Day course that covered the ISO 9001:2000, plus the supplemental TS16949:2002 requirements including the "core tools", such as APQP, PPAP, MSA, SPC Automotive "Blue Books" formerly associated with QS9000.
