Migration (Transition) to ISO 13485:2016 from ISO 13485:2003

Dear all
I am starting the ISO 13485:2006 Migration from 2003.I have done a gap analysis between 2003 and 2016 and with respect to our processes and documents.
It will be great if the experts can share your inputs / gap analysis or related documents so that I can ensure that I don't miss out anything as well as do it diligently
