Reliability Centered Maintenance - Nowlan & Heap's AD-A066579 (1978)

I am looking for a specific reference in Nowlan & Heaps RCM document AD-A066579 (1978) which indicates that 60% of all USAF maintenance was CAUSED by maintenance.

If anyone could help me find this specific reference and its location in the report, or correct the figure should 60% be wrong, I would be most grateful.
