we can consider the following also :
1- the verifying of effectiveness should be stated from section / department who initiate the sources of findings/NCs , --
2- the action plan could be initiated not only for finding or NCs discovered , but for improvement & development team as well ,
3- culture of the organization could extend to make Quality circles , improvement teams ; Ishikawa / fishbone root causes analysis ,--etc -- therefore we can find that the organization culture can prevent the occurrence of many NCs rather than detecting or discovering finding or a deviation
