IMO, if it's been going on a long time, there should be a trail of internal audit non-conformance's ,and those audit results being reviewed by the appropriate management, so it doesn't come down to "we had no idea this was happening". The internal audit system is the first line of defense of preventing things from eroding into a major QMS system malfunction.

In the real world, the overwhelming majority of internal audits are valueless exercises. Poorly selected and underdeveloped internal auditors cannot find a real significant discrepancy even if it sat on the auditor’s lap and call them daddy.

yes Sidney. The OP doesn't need an "internal audit". they need to specifically (inspect) audit (100% as initial containment) the data collection process for all characteristics that are required to be documented...

If it’s been going on a long time, one might question the value of the exercise. I mean how can this data not be collected and the business still function.
