Survey Says?

To All of the Denizens,

The debate on the customer survey in another current thread, (and the fact that we are currently designing one) leads me to this question?

What questions do you want on a customer survey, and why. Depending on the responses, I may do a poll to find the most popular.

My candidates are: (Rate 1 (poor) thru 5 (excellent))

Product Quality
On Time Performance
Communication
Response to Reject Reports
Overall Value

(Additional Comments)

Any others?

What about price. I consider "overall Value" to include price, delivery and quality.

I tried the Additional comments line - on a past survey
for us it didn't work out all that well...

Got information like - "we would be more satisfied if you didn't send us surveys to fill out."

One addition that we seemed to get a good overview of was
"do you find our employee contacts available/knowledgable"

One of my clients uses this interesting question in the surveys sent out to customers: "Tell us the rating you have given to your BEST OTHER VENDOR."

Reason: You come to know if your customer rates somebody (not necessarily your competitor) higher than you and then try to benchmark against this other vendor.
