If we assume that HS=Harm it does not implies that there was no "the last event'. It just means that all events leading to harm have happened.
But it does not make any of them inevitable. We are still able to break the chain!

That is true, and it would rely on all events being a part of the sequence of events leading to the hazardous situation.

This makes P2 simply a case of medical probabilities and picking an appropriate harm out of the list, as we know that the probability of harm and severity of harm are on a continuum. You can stub your toe and die, but you're more likely to just have some minor discomfort.
