I worked for company with laminated +10000 work instructions, +10000 product inspection instructions, +1000 packaging standards, +2000 maintenance standards, ... so imagine ... No, don't even imagine that.
Each document before distributing it to users has to be somehow reviewed and approved.
It does not mean that footer on initial or last page of document has to include table with 3 columns - created by, checked by (review) and approved by (approve). This is nor effective nor efficient. Let there 2 columns only - for document creator and approver.
