But just to flush out the thread, another good tool (for those using the Microsoft 365 ecosystem) is Microsoft Forms.
At least it's great for certain applications: where the form is intended to be completed in one go, and not later edited).
It's super easy to create shareable, reusable forms, and all submitted data goes directly into a spreadsheet, thus facilitating data-analysis.

Another low-budget solution is to use the "developer" tools in your word processing software (Word, OpenOffice,...) to create fillable forms that you can export to form PDFs (so end-users of the forms can't mess with the fields and formatting).
