Management of Change - MOC Procedure and Forms - Engineering, Project, Safety Systems

Hi All

I've to create (urgently) a MOC procedure and an MOC form and am looking to capture element such as engineering, project, safety systems, bulidings, plant etc

We currently only have an Engineering Change Request process so we want to capture it all under an MOC process

Does anyone have any examples of an MOC form they could share so I can use as a starting point. Looking for something that obviously takes in to account the elements with a series of questions that the requester completes prior to submitting to the signatories for sign off eg Enginnering manager, HSEQ manager, project manager etc

Any start point would be greatly appreciated
