Hazard Identification and Risk Assessment 4.3.1

Hi, hope someone can help me with this...I have just been given the job of implementing 18001 at work, having successfully just achieved 14001.

I can see the crossover for most points within 14001 and 18001, however I am having a little trouble defining the hazard identification and risk assessment register.

Please would someone look at the attached draft example I have developed for my HIRA register and comment? I will of course draw up a methodology and rank the risks.

Unlike my 14001 aspect register, I am creating a HIRA register for each office we have - is this necessary?

Should I provide a risk rating for all operating conditons as well?

Yes, go point by point through clause 4.3.1 and do exactly as it states. This includes potential hazards and the risks that could be associated with them as well including changes to anything that could create hazards to employees or others in the workplace.

As for other replies do involve others and make sure you involve the H & S specialists as well. These are the guys and gals who can make sure your risk register is appropriate based on actual experience in the industry and that your legal obligations are satisfied - fairly substantial in the UK.

Layout looks good. The severity figures need to be looked at, though, as the two hazards you have identified are considerably more severe than you have recorded.

Don't mix severity with likelihood. For example an electrical shock can cause death - it doesn't mean the risk is high - because of all the controls you have listed.

Only if the risks are different. I would be surprised if the severity changes but the likelihood might due to different conditions / controls.

Again - it depends! You need to consider operating environment, routine, non-routine and emergency situations. If the hazards and risks are all the same then the one rating can cover all.

Bear in mind also that the risk rating should be the driver for your objectives and targets. I.e. you focus your improvement activities on the areas of most potential benefit in H & S!

Depending on the ingress and egress and how the cleaning process is executed, you may have to consider it (at least in the US) a confined space entry process/operation....I am sure others can add to or correct this post

I would think that a lot would depend on the dimensions (not volume) of the tank, and whether you're cleaning the inside or the outside, or both.

If it's full, drowning

Inside it would be considered a confined space in the USA/Canada

Outside at the minimum a fall hazard in the USA/Canada

And then you would have the hazards of the chemicals used in the cleaning
