Sub : Detection rating

1. The FMEA manual Third edition Page 53,Table 8 says detection ranking of 2 can be assigned where "error detection is done in subsequent operations by multiple layers of acceptance: supply,select,install,verify".Can anyone please tell me what it means with an example. Likewise what detection ranking must be given in cases where "setting approval" is done to detect the failure and what ranking in the case of receiving inspection?

2. Does any of you use a format for collecting the nonconformance data and customer complaints data for assigning occurence rating?If so, can you share it with us?

3.How frequently do you review the nonconformance data and customer complaints data to change the FMEA.

4.How many of you have revisited the FMEA's,that were prepared earlier, in line with the 3rd edition requirement.

As a quickie to #1, Company X receives part A and does no inspection. Part A is one component of a sub-assembly. The 'item' contains 4 sub-assemblies. When complete, each sub-assembly is tested. If Part A was defective the sub-assembly test would not work thus the part was indirectly 'inspected' after its incorporation in the sub-assembly. On the same note, it may be final test of the complete 'item' before it is 'indirectly' tested.

More later - gotta get back to 'work'....

On point 3 -
I would link the frequency of review to repetitive issues. If you do find that some issues are coming up time and again, it is the right time to review FMEA.
