Oh, I understand. A person who accepts training as a prevention control is either unconsciously incompetent, or conciously incompetent, but not, you know, actually incompetent. Thanks for clearing that up.

I'll answer that with another question: isn't prevention of nonconforming conditions a prime part of the purpose of operator training? How this might apply to a given operation or process is dependent on individual circumstances, but you shouldn't have to think very hard to envision the likelihood of a potential failure cause being significantly reduced, or prevented altogether, by indoctrination.
