Creating a xlsx Customer Complaint file to track Complaints

My boss just asked me to create a file or matrix to were I can also track specific failures to each company(customer). Any suggestions would greatly be appreciated. Not that great in excel.

I like to use tables and pivot charts to organize my data in excel. Here is a simple example of something that I have used. Pivot charts can really help to organize your data into useful information that is highly shareable (is that a word?).:)
